Hello, this is Diva Plastic Surgery.
Many of you may be concerned about the changes in your eye appearance as skin elasticity decreases and wrinkles begin to form one by one due to aging.
In particular, aging under the eyes is often the first area to show signs of aging, leading to concerns about deep dark circles, fat, and wrinkles that make one look older and dull.

It is a natural phenomenon for skin elasticity to decrease and wrinkles to appear one by one due to aging.
Horizontal wrinkles sag diagonally, and taut skin becomes loose; everyone experiences this at a certain age.
The area where these signs appear first and most prominently is under the eyes. This is because the skin layer is thin and it is an area where facial expressions are frequently made, leading to a lot of movement and making it prone to wrinkles.
Dr. Kim Jong-myung (a full member of the Korean Society of Plastic and Reconstructive Surgeons) of Gangnam Diva Plastic Surgery explained, ‘When skin sags and fat accumulates, the eyes themselves can look more hollow and tired, and the overall impression can appear dull, so thorough daily care is important. However, if daily care alone is insufficient, it is necessary to improve it through appropriate methods.’
◆Lower Blepharoplasty…Improves both skin and muscle sagging
For aging under the eyes, one can consider correcting under-eye wrinkles and removing fat. Among these, ‘lower blepharoplasty’ is one of the representative methods that improves both skin and muscle sagging.
Lower blepharoplasty simultaneously improves under-eye wrinkles and muscle laxity, making it a suitable mid-life eye surgery method for those who ‘look older than their age’ or ‘middle-aged and older individuals whose under-eyes clearly show signs of aging’.
Another major advantage is that since a minimal incision is made in the inner fascia of the lower eyelid, scars are not visible after recovery.
Dr. Kim Jong-myung stated, ‘After making a long incision along the lower lash line to the outer corner of the eye, the sagging skin and protruding fat are appropriately removed, considering the condition of the loose skin and the shape and amount of fat. By pulling up the loose and inelastic muscles and firmly fixing them to the fascia before suturing, one can achieve brighter and younger-looking eyes.’
◆Under-eye Fat Repositioning…Flattens the under-eye area
If excessive under-eye fat is a concern, under-eye fat repositioning can be helpful. As aging progresses, the fatty tissue under the eyes thins, and the eyelids gradually sag downwards, causing the fat layer to bulge forward.
This can make one look angry or tired, and if dark circles also form, creating shadows, the area can appear dark, necessitating correction.
If skin laxity is not severe, an incision is made inside the eyelid, and the protruding fat pockets are found and repositioned to the sunken areas. If the sagging is severe, the inner conjunctiva of the eye is opened, and fat is removed to flatten the under-eye area more effectively.
Dr. Kim Jong-myung said, ‘The skin under the eyes is a very thin and sensitive area, and the speed of aging and the degree of skin sagging vary depending on age and skin type. Therefore, surgery must be performed using an appropriate method based on a meticulous analysis. Carefully assessing the amount of fat, skin wrinkles, and degree of sagging to perform 1:1 customized surgery is one of the ways to enhance treatment effectiveness and satisfaction.’
