Bulbous nose surgery is a procedure to reduce a large and thick nasal tip. Usually, a bulbous nose refers to a case where the nasal tip is not well-defined and is large. Since a bulbous nose often has wide nostrils, alar reduction and bulbous nose surgery are performed together. To distinguish them, bulbous nose surgery reduces the bluntness of the nasal tip, while alar reduction surgery reduces the width of the nostrils.

Q. My nose is very blunt and fleshy, and I want to have nose surgery, but I heard that improvement is difficult with general nose surgery. How can I increase satisfaction?

Usually, when the nose area has a lot of flesh and is thick, people only think it's due to excess flesh. However, in reality, large and widely spread cartilages at the nasal tip account for a significant portion. In such cases, the surgery first involves tying the cartilage to make the nasal tip more defined. After that, it is advisable to perform the surgery by further stacking cartilage.

And if the nose has a lot of flesh, surgery to remove the perichondrium, muscle fibers, and fat inside the nasal skin is also performed to generally sharpen the contour.

Q. Is it possible to use autologous tissue for bulbous nose surgery? Are there any concerns about side effects from artificial implants?

Autologous tissue can be used for bulbous nose surgery. Typically, for a bulbous nose, surgery involves tying the nasal cartilage at the tip, and then ear cartilage or septal cartilage is stacked to increase the overall height. For the nasal bridge, implants are usually used for height, but this part can be raised with autologous tissue using your own dermal fat graft. However, since dermal fat grafts provide a less defined look compared to implants, it is effective to combine it with osteotomy to make the nasal bridge appear higher.

It cannot be said that artificial implants have no side effects. However, if the surgery is performed to minimize side effects, and careful observation is maintained after surgery with appropriate responses to any issues, most cases can be completed without concerns about side effects.

Q. Can men with a high nasal bridge and a large nose also improve their impression with bulbous nose surgery?

Usually, a high nasal bridge and a large nose give the impression of a large nose rather than a high nose. In such cases, when viewed from the side, despite being high, the nose bone and nostrils are wide, so there is no feeling of a high nose. In such cases, if the nasal bridge is sufficient, an implant is not used, and only the wide nasal area is narrowed through osteotomy. For the nasal tip, wide cartilage is tied to reduce the tip.

Typically, a large nose also has wide nostrils, making the impression appear dull. Therefore, reducing the wide nostrils with alar reduction surgery yields good results.
