However, more recently, a large, round, blunt, or wide nasal tip has become synonymous with a crude or unsophisticated image. Consequently, many people wish to transform their ‘bokko’ – which can make them look sullen – into a sleek, refined nose to overcome their complexes and enhance their appearance.

Q. Bokko rhinoplasty and alar reduction surgery seem similar. Is there a difference?
Bokko surgery is a procedure that reduces a large and thick nasal tip. Typically, a ‘bokko’ refers to a nose with an indistinct or large nasal tip. Since a bokko usually also involves wide nostrils, alar reduction and bokko rhinoplasty are often performed together. To differentiate, bokko surgery primarily reduces the blunt appearance of the nasal tip, while alar reduction reduces the width of the nostrils.
Q. My nose is very blunt and fleshy, and I want rhinoplasty, but I've heard that general nose surgery might not be enough. How can I achieve a satisfactory result?
People often assume that a fleshy and thick nose is simply due to excess flesh. However, in reality, large and widely spread cartilages at the nasal tip play a significant role. In such cases, the surgery typically begins by clearly defining the nasal tip cartilage, often by tying the cartilage. Afterward, it is advisable to perform additional surgery to build up the cartilage further.
For a fleshy nose, a procedure to remove the perichondrium, fibrous tissue, and fat from inside the nasal skin is also performed to create a more defined overall contour.
Q. Is it possible to use autologous tissue for bokko rhinoplasty? Are there any concerns about side effects from artificial implants?
Autologous tissue can be used for bokko rhinoplasty. Typically, for a bokko, the surgery involves tying the nasal tip cartilage and building up ear cartilage or septal cartilage to increase the overall height. For the nasal bridge, implants are usually used for height, but this can be achieved with autologous tissue using dermal fat grafting. However, since dermal fat grafting provides less definition compared to implants, it is effective to combine it with osteotomy to make the nasal bridge appear higher.
It’s impossible to say that artificial implants have no side effects. However, if the surgery is performed to minimize side effects, and careful observation and appropriate responses are made in case of problems post-surgery, most cases can be completed without concerns about side effects.
Q. Can men with a high nasal bridge and a large nose also improve their impression with bokko rhinoplasty?
Typically, a high nasal bridge and a large nose tend to give the impression of a large nose rather than a high one. In such cases, even if the nasal bridge is high when viewed from the side, the wide nasal bones and nostrils prevent it from appearing high. For such cases, if the nasal bridge is sufficient, an implant may not be used, and only the wide nasal area is narrowed through osteotomy. The nasal tip is also reduced by tying the wide cartilage.
Usually, a large nose also involves wide nostrils, which can make the impression appear dull. Therefore, reducing the wide nostrils through alar reduction surgery can yield good results.
