In such cases, it's not merely about excising the sagging eyelid skin; rather, the sagging skin, fat, muscle, and subcutaneous tissue are excised depending on the degree and area of sagging.

◆Upper Eyelid Surgery…Addressing the fundamental cause of drooping eyelids, etc.

Upper eyelid surgery involves making an incision along the double eyelid line, removing sagging skin and protruding fat, and suturing the levator palpebrae superioris muscle to the skin.

Additionally, a minimal incision in the inner fascia of the lower eyelid ensures no visible scars, and by simultaneously pulling the skin and muscle upwards, the under-eye area can be tautly lifted.

When combined with lateral canthopexy, which fixes the sagging skin and muscle to the periosteum on the outer side of the eye, severe under-eye sagging that is difficult to improve with lower blepharoplasty alone or ectropion can be corrected.

◆Under-Eye Fat Repositioning…Improving dark circles, etc.

If the degree of under-eye fat sagging is not severe, and there is either excessive or significantly insufficient fat, under-eye fat repositioning can be applied. By minimally incising the inner fascia of the eyelid and appropriately repositioning the fat, volume can be created under the eyes.

This can improve dark circles and create a bright, youthful eye appearance by forming 'aegyo-sal' (under-eye fat rolls).
