Thoughts on Middle-Aged Eye Surgery: Upper Blepharoplasty, Lower Blepharoplasty

Today, I'd like to share my thoughts on eye surgery for middle-aged individuals.

This includes upper blepharoplasty, lower blepharoplasty, and dark circle treatment.

The ‘eyes’ are where people's gaze lingers the longest, making them a focal point for overall impression.

Especially with age, as skin elasticity decreases, various changes occur around the eyes.

The thin skin of the eyelids moves incredibly fast countless times a day, and this accumulated fatigue leads to skin sagging.

Furthermore, weakening eyelid muscles can cause ptosis, where one uses their forehead to open their eyes. Sagging eyelids can also narrow the field of vision, cause eyelashes to poke the eyes, leading to tearing and skin irritation.

◆ Upper Blepharoplasty

Upper blepharoplasty removes sagging upper eyelid skin and protruding fat, connecting the levator palpebrae superioris muscle to the skin to create clear and refreshed eyes.

‘It's not just about simple removal; we meticulously identify where the sagging is more prominent and correct it, which not only provides therapeutic effects but also a more youthful appearance.’

When the upper eyelids are deeply sunken, it can make one look very tired and hollow. Due to aging, the fat in the eyelids gradually depletes and sinks. Depending on the degree of indentation, delicate fat grafting can create a natural eye shape.

◆ Lower Blepharoplasty

Lower blepharoplasty involves removing excessive under-eye fat, excising sagging skin, tightening the loosened muscles, and firmly fixing them to the fascia.

It is known to flatten uneven under-eye skin while also improving skin wrinkles.

◆ Lower Blepharoplasty + Lateral Canthoplasty

‘Excessive muscle resection or extending the incision line towards the inner eye increases the risk of nerve damage, potentially leading to ectropion (eversion of the eyelid).’ He added, ‘By performing lateral canthoplasty along with lower blepharoplasty, and firmly fixing the sagging skin tissue and muscles around the eyes to the periosteum, the possibility of side effects is reduced. This also helps improve severe under-eye sagging that is difficult to correct with lower blepharoplasty alone. Such a combined surgery can lead to rapid improvement.’

◆ Dark Circle Improvement

Excessive accumulation of under-eye fat can cause dark circles, making the overall impression dull and appearing older.

If it's simply sagging fat, it can be easily corrected with simple under-eye fat removal. If the fat is bulging and the tear trough is deeply indented, under-eye fat repositioning can correct it, and you can expect dark circles to be alleviated simultaneously.
